There is a $25 USD
non-refundable Application and Enrollment fee for each level 1 of
Certification.
The Program Fee for Practitioner
Coach Certification Level is $300 USD and includes all
the training and tutorial materials, examination processing,
correspondence, and 10 hours of instructor coaching. The
DNA Book and Activation Kits are not included.
Applications need be
submitted for each additional level and must be accompanies by $15
15 USD processing fee.
Should you decide to advance
to the status of Facilitator
Instructor Certification level 4 , the Program Fee of $450
USD; and includes all the facilitator training, tutorials, examination
processing, correspondence, and 15 hours of instructor coaching. The
Application Fee is reduced to $15 USD

Please be advised that you
are entering a business relationship and must uphold all agreements,
copyright and trademark laws. A 10% Royalty Fees applies to all royalty
based activities and services as outlined in the Certification
Agreements.

certification and fees paid shall be pro-rated.

Agreement of Ethical DNA Activations
Addressed to the General
Public
(Revised 30 June, 2006)
his Agreement constitutes my
comprehensive understanding and nature of the potential effects of
performing DNA Activations upon
myself and their effects upon others.
I have been properly
informed that DNA Activations can stimulate physiological,
psychological, emotional, and spiritual changes and transitions, both
in myself and others. This information has been
conveyed to me via any one or any combination of the following: by
reading
the book Change Your DNA,
Change Your Life!, by attending a DNA
Activation Seminar, from DNA Workshops or Courses, DNA educational
handouts and Kits, or verbally.
It is agreed and understood
that the Oughten
House Foundation’s has clearly stated that
“…anyone can self administer
or perform a DNA Activation upon oneself. However, it should not be
performed
on others unless properly trained as a Certified DNA
Practitioner.” …It’s
Oughten House Foundation’s belief and policy that each
individual be accountable for his or her own healing and
self-empowerment. Performing DNA Activations upon another requires
professional awareness, accountability, and skills training.
The primary reasons
substantiating this ethic of accountability is that only through
extensive Certified Training does a practitioner fully understand the
DNA Activation process, effects, and ongoing transition to properly
guide the client. DNA Activations are,
in affect, a most sacred ceremony and a major healing process which
requires accountability and support from a healing professional.
Secondly, the
structure of the Foundation and availability of the Practitioners can
serve as a professional support, counseling, and long-term coaching.
Thirdly,
it avails the process to allow new information and healing techniques
to
be properly and quickly passed from the Foundation, to the Certified
Practitioner,
and onto the Client.
The Foundation truly
encourages achieving results in a dignified manner and to uphold integrity and honesty. It is dedicated to bring
forth this information to the public,
and to work with love and joy
from the heart.
In this pursuit, the
Foundation request that
the individual pledge their allegiance to uphold this Code of Ethic and
not perform DNA Activations upon others unless properly Certified.

Agreement for DNA
Practitioner Certification
Level 1
(Effective 1 June 2006)
This Agreement is between
Oughten House Foundation, Inc., a non-profit, tax exempt corporation
including: its copyright owners, sub-contractors, affiliates, and
licensees, with offices at Box 74 Binghamton, NY 13903 USA (herein
after known as the Foundation)
and

whose office is located at
________________________________________________ (herein after known as the Practitioner).
Whereby,
The Practitioner understands
that Program is the property and product of the Foundation and must
abide by the codes and procedures set forth by the Foundation.
The business relationship
set forth herein constitutes the understanding that the Foundation
acknowledges your competencies as a Certified DNA Level 1 Practitioner,
as stipulated herein, will stand
firmly behind you, and will continue to augment your Practitioner
competencies as resources permit.
The Practitioner thoroughly
understands that a
flat 10 percent royalty be paid in a timely manner to the Foundation
for
all activations and seminars that Certified Practitioner renders. The
Practitioner also understands that an appropriate fee be charged to the
client and/or
training participant
to mutually satisfy both Practitioner and
the Foundation.
It is agreed and understood
that the Oughten House Foundation’s Mission embraces the
concept of the “Genetic Grid”—a worldwide
endeavor to disseminate and provide very unique DNA Activations to all
interested people.
As
a Certified DNA Practitioner I agree to:
1.
Work
as an Independent Representative (non-employee) of Oughten House
Foundation, Inc.; maintain required legal state, federal, and
provincial licensing and tax responsibilities.
2.
Uphold the Codes of Ethics,
Professional Standards, and principles of the Foundation.?
3.
Adhere
to the criteria set forth in the Details of DNA Certification Program
Overview and all certification related
documents within the Pre-Certification Package.
4.
Maintain
an “ACTIVE” status which implies activating at
least five people and generating at least $50 USD of business from all
sources, for the Foundation every 90 days (a fiscal quarter).
5.
Purchase Practitioner
Manuals for DNA Activations and Seminars from Foundation.
6.
To serve, coach, and assist
in the activation and healing of new and existing clients based on the
LEVEL ONE of Certification as follows: --perform First & Second
DNA Activations plus Introductory Seminars and Small Groups with a
maximum of five (5) people.
7.
Perform
DNA Activations only with individuals and in small gatherings (maximum
of 5 people). Agree not to train, facilitate, or perform in larger
groups of more than 5 people, which then becomes the role of a
Certified DNA Facilitator.
8.
Earn
35% commission income from selling the Foundation’s Products
and 10% on initial referral commissions on
Services
provided by affiliates of the Foundation. This does not include
“Hosting.”
9.
Earn
income and charge a reasonable
fee for Practitioner Services
rendered to clients including: coaching, performing DNA Activations and
DNA Seminars.
10.
Encourage
potential clients to purchase and use
the DNA book, DNA Activation Kits, articles, and research documents
that you have on inventory or from OHi’s website.
11.
Abstain
from being competitive with the other Foundation practitioners,
products and services.
12.
To
generate signed testimonials and copy such to the Foundation.
13.
Keep accurate records of any
work performed on
behalf and/or associated with the Foundation.
14.
Award
“DNA Activation Medal ” for each level of DNA
Activation properly performed.?
15.
Introductory
Seminar Packets and Medals ,
which include Corporate seal, are purchased from the Foundation at $2
USD each per each person.
16.
Forward
mailing addresses and occupation of clients activated in order to
develop a “confidential” worldwide database (i.e.,
“client documentation”), or as otherwise
negotiated.
17.
Help
propagate and market the expansion of the Foundations’
“Genetic Grid” mission, educational programs, and
future material including the 5th
DNA and subsequent Activations.
18.
All
retail Program materials shall be ordered from the Foundation, unless
otherwise arranged. The Foundation will
sell all educational products at a distributor wholesale discount of
35% to the Practitioner.?
Vendor products
discounts
varies, and usually are discounted around 20 percent.
19.
Practitioner
shall reference, market and sell the products and services of the
Foundation under the trade names, trademarks, and logos of the
Foundation. Practitioner agrees not to reproduce any of the
Foundation’s Products unless written permission is granted.
20.
Practitioner
will report any changes suggested and keep current any information that
pertains to the Foundation. No
new or alternative generated information or materials related to the
Program shall be used unless approved by the Foundation.
Oughten
House Foundation, Inc., agrees to:
1.
Post
Practitioner’s personal contact data on
the OHi Website.
2.
Provide
local leads, referrals, and support Practitioner within his/her local
community.
3.
Provide business development
opportunities as available.
4.
Advertise Practitioner at
National and International Conferences.
5.
Provide
generic marketing and educational materials
6.
Research
and disseminate new materials as available.
7.
Provide
technical support about the Program and Client Support
8.
Promote
and expand the Educational Programs and DNA Healing Project worldwide.
9.
Develop
new educational programs and venues to further your competencies.
10.
Provide
professional resources as available.
Common
Concerns:
1.
Practitioner
warrants and indemnifies Foundation of all encumbrances,
judgments, liabilities, litigation’s, which the Practitioner
has been or involved in or may be subject to in the future.
2.
Foundation
warrants and indemnifies Practitioner of all encumbrances,
judgments, liabilities, litigation’s, which the Foundation
has been involved in or may be
subject to in the future.
3.
The
Foundation has the right to audit and monitor its programs without
notice at its own expense.
4.
Termination
by either party provided a 15-day written notice is given. The
Practitioner fully understands that in the event of
termination or non-renewing annual certification, then the Practitioner
will
completely cease in the capacity of the Certified DNA Practitioner of
the
Foundation.
5. It
is understood that any violation of the terms of this Agreement may be
just cause for my immediate termination as a Certified DNA Practitioner
by the Foundation, and is fully aware of the legal consequences of such
a violation, including legal, attorney, and court fees.
6.
The
Remote Program Certification Fee of $300 USD includes initial Level (1)
training, training materials, examination processes, e-mail
correspondence, instructor participation, and 10 hours of Senior
Instructor Coaching.
7.
DNA
Practitioner Certification requires annual renewal, additional
in-service training requirements, and a reasonable renewable fee of $50
USD. Certification and renewal fees are non-refundable.
8.
This
agreement shall be construed and enforced according to the laws of the
State of New York, in the United States
of America.
This agreement can only be
modified in writing
and acceptance by both parities.

LICENSe
This
Licensing Agreement is between Oughten House
Foundation, Inc., a non-profit, tax exempt corporation with offices at
Box 74, Binghamton, New York,?

after known as the Licensor/Foundation) and

(herein after known as the Facilitator ).
This
Agreement proclaims
that the
DNA Certified Facilitator Level Four (4) named herein is an
“Independent Subcontractor” of the Licensor. The
Facilitator has “active” status and experience as a
Certified DNA Practitioner of the Foundation; has successfully passed
the required Remote or On-Campus Facilitator Training Program
requirements, and has paid all fees.
The
Facilitator has the license to host and facilitate only the
Self-Empowerment Institute Programs listed below; and is eligible to be
hired and properly compensated by the Foundation, an Affiliate
organization and/or Host to perform such facilitations.)

Whereby,
The
Licensor hereby grants
the Facilitator the non-exclusive license to market and perform the
following:
- DNA
Activation Introductory Seminars
- FIRST,
SECOND, THIRD, &
FOURTH DNA Activation Seminars
- DNA
Activation Experiential Techniques Training--DNA Course #1
- Living
Life Positively
and How It Is Done (Expansion & Rejuvenation Course)--DNA
Course #2
All
Training Courses for instruction and/or facilitation are herein known
as the WORK EVENT, in
conjunction with Licensor. Amendments to this Agreement will be used to
add or delete WORK EVENTS.
The Facilitator understands
that the WORK EVENT is the property and product of the Foundation and
that each WORK EVENT reflects the integrity and
name of the Foundation. Accordingly, the Facilitator must abide by the
codes and procedures set forth by the Foundation.
This License does not pertain
to the compensation arrangements for Facilitator assigned to teach
classes that are generated and scheduled by the Foundation’s
home office or any Affiliate Organization or Host. The following items
constitute the full extent of the License Agreement:
On
the part of the Facilitator:
1.
Be
compensated for delivering high quality standards for course content
and
learning.
2.
Every
PLANNED Work Event
must be registered and posted by the Foundation at least 60 days prior
to class date, unless otherwise agreed.
3.
Each
WORK EVENT must have a value set for its tuition. The Facilitator is
responsible for setting the tuition, based on the standards of living
and economics of that
region. The Foundation can advise Minimums and Maximums for tuition.
The Foundation’s standard use in the UNITED STATES equates to
approximately
$95 per day of training, per participant. Facilitator may lower or
surcharge this tuition based on projected higher expenses.
4.
Guarantee
participant’s satisfaction: when conducting the WORK EVENT,
the Facilitator agrees
to abide by the Ethics Policy, the Practitioner Certification
Agreement, and other Foundation’s Policies.
5.
The
Facilitator agrees to pay a Program Fee of $450 which includes a one
(1) time license set-up fee of $50 for all office administration
activities, the SEEP Set-Up Kit, future updates of the SEEP, potential
Facilitator assignments, referrals, leads generation, and initial
posting on the INTERNET.
6.
A Program
Royalty Fee of 10%
shall be paid to the Foundation for each WORK EVENT. Program
Royalty Fees shall be
paid based on GROSS Revenues prior to expenses.
7.
Facilitator
Guides: one
Facilitator Guide must be purchased and used for each Work EVENT. Each
seminar, workshop, course, and advanced course has an Facilitator
Guide.
8.
Facilitator
Guides cost
between $35-$75 and include all updates and future enhancements,
overheads, and curriculum for the specific WORK EVENT. Only one
Facilitator Guide is needed per WORK EVENT. It is highly recommended
that the Facilitator become thoroughly familiar with each Facilitator
Guide several months prior to facilitating the WORK EVENT (Seminar,
Workshop, or Course).
9.
Facilitator
pays own travel, diem, lodging, material reproduction, etc.
10.
Course
Participants must
purchase all “Primary” educational and course
material from the Foundation at the then current posted retail price.
Foundation will sell all educational products at a
wholesale discount of 35% to the Facilitator for resale at each WORK
EVENT, if so required.
11.
Any
copyright material used shall be cited and the copyright source
revealed and properly affixed on each page of the handout.
Any copyright
infringements and/or judgments on the part of the Facilitator shall not
liable or burden the Foundation.
12.
Foreign
Translations of training materials are subject to International
Copyright Laws and remain the property of the Licensor. Fees for
translation are subject to a separate contract.? Retail
sales of translated materials are subject to a 15% Program Royalty Fee
to the Foundation, however, all
other proceeds from these materials go directly to the Facilitator.
13.
All
course materials shall
be ordered from the Licensor, if so required.? Any
new Facilitator generated materials related to the course shall be
shared with the Licensor.?
Facilitator as the
option to market the products and services of the Licensor under the
trade names, trademarks, and logos of the Licensor and not market any
competitive products.
14.
Maximum
attendance for each is limited to a ratio of 16 participants for each
Facilitator. Facilitator agrees to use an assistant(s) for larger
groups based on the 16 participant rule, whenever possible.? Assistants
must hold at
least a DNA Practitioner’s Certification. Compensation for
assistance
is the sole discretion of the Facilitator.
15.
On-site
enrollments must be reported to the Foundation’s Center.
Strict financial accountability
must be honored by both parties of this Agreement. Facilitator agrees
to use the Licensor’s Enrollment Forms supplied and to
furnish a copy
to the Foundation within three (3) days after the end of the WORK
EVENT.
16.
Facilitator
has unlimited use to promote the educational products of the Licensor
providing that the Facilitator remains in good standing and has renewed
the Licensing Agreement.
17.
Facilitator
agrees not to reproduce any of the Foundation’s Products
unless written permission is granted.
18.
The
Foundation has the right to audit and monitor its programs without
notice at its own expense.
19.
Termination
by either party provided a 30-day written notice is given. The
Facilitator fully understands that in the event of termination or
non-renewing annual certification, then the Facilitator will completely
cease in the capacity of the Certified DNA Facilitator of the
Foundation.
20.
It is
understood that any violation of the terms of this Agreement may be
just cause for Facilitator's immediate termination as a Certified DNA
Facilitator by the Foundation, and he/she is fully aware of the legal
consequences of such a violation,
including legal, attorney, and court fees.
21.
Facilitator
warrants and indemnifies Licensor of all encumbrances, judgments,
liabilities, litigation’s, which the Facilitator has been or
involved in or may be subject to in the future.
On
the part of the Licensor:
1.
The
Foundation will generate primary marketing information and advertising
copy information and originals for the Facilitator to help launch the
promotional campaign of the Facilitator including: flyers, brochure,
and other media used by the Foundation.
2.
The
Licensor agrees to
develop and generate state-of-the-art educational programs for sale,
and to maintain the highest educational standards of their programs.
3.
Diplomas
of Completion stating
the names, course
titles, will be generated by the Foundation and mailed to the
participant
or returned to the Facilitator for Co-Signature and distribution.
Certificates will be issued solely by the Licensor upon receipt of the
completed Enrollment Forms and financial documentation. A $2 USD
processing fee is
required for each Certificate or Diploma issued by the Foundation.
4.
The
Foundation has the right to audit and monitor its programs without
notice at its own expense.
5.
Licensor
agrees to submit leads to the Facilitator within the Facilitator
’s established region.
6.
Licensor
warrants and indemnifies Facilitator of all encumbrances, judgments,
liabilities, litigations, which the Licensor has been involved in or
may be subject to in the
future.
7.
The
Licensor will maintain a database of all attendees and records of all

information mutually with Facilitator s on a “Confidential
basis only.”

This
agreement shall be construed and enforced according to the laws of the
State of New York.
This
Agreement shall be automatically renewed annually unless terminated.?
This agreement can only be modified in writing and acceptance by both
